Takes about 15 – 20 minutes for examination
£50 fees (cash only)

HGV and LGV drivers should undergo a medical examination before applying for a HGV / LGV drivers licence or at periodic renewals. You will be examined by our doctors based on DVLA D4 standards and provide you with a completed D4 medical form to send it to DVLA.
Things to bring for your HGV/ LGV driver medical appointment:
- D4 form sent by DVLA
- Photo ID (Driving Licence or Passport)
- Eye glasses or eye lenses (if you wear them for driving)
- Medication list (if you are taking any)
- Medical notes / letter from GP (if you have any medical conditions)
